Test Results
If any tests are taken, it is your responsibility to telephone the surgery for the results. Please only telephone between 1.00pm and 3.00pm. If you wish to discuss the results, please request a routine telephone conversation with your GP or Nurse.

Cancelling your Appointment
If you are unable to attend an appointment with one of the doctors or nurses, please telephone either surgery.

PRIVATE MEDICAL EXAMINATION AND FEES

Medical examinations such as elderly driver, insurance medicals, HGV and employment medicals are not included under the NHS.

Please see the receptionist who will advise you of the fee involved. Appointments will be arranged with the doctor via reception. There are also fees for private medical reports and certificates. The current list of fees is on display in the waiting room.

Many illnesses can be prevented through healthy living. Remember you are responsible for your own health. Appointments can be made with any of our practice nurses to discuss your concerns.

Vaccination Schedule

At two months old:

Diptheria, tetanus, pertussis (whooping cough), polio and Haemophilus influenzae type b (Hib) (DTaP/IPV/Hib) - one injection
Pneumococcal infection - pneumococcal conjugate vaccine (PCV) - one injection

At three months old:

Diptheria, tetanus, pertussis (whooping cough), polio and Haemophilus influenzae type b (Hib) (DTaP/IPV/Hib) - one injection
Meningitis C (meningococcal group C) (MenC) - one injection

At four months old:

Diptheria, tetanus, pertussis (whooping cough), polio and Haemophilus influenzae type b (Hib) (DTaP/IPV/Hib) - one injection
Meningitis C (meningococcal group C) (MenC) - one injection
Pneumococcal infection - pneumococcal conjugate vaccine (PCV) - one injection

At around 12 months old:

Haemophilus influenzae type b (Hib) and meningitis C (Hib/MenC) - booster dose in one injection

At around 13 months old:

Measles, mumps and rubella (German measles) (MMR) - one injection
Pneumococcal infection - pneumococcal conjugate vaccine (PCV) - one injection

Three years four months to five years old (pre-school):

Diphtheria, tetanus, pertussis (whooping cough) and polio (dTaP/IPV or DTaP/IPV) - one injection
Measles, mumps and rubella (German measles) (MMR) - one injection

13 to 18 years old:

Diphtheria, tetanus and polio (Td/IPV) - one injection

Travel Information

Travel chats and immunisations are available by appointment with the nurse at Locks Road and Whiteley Surgery. Please ensure you make this appointment at least eight weeks before travel.

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. There is further information about countries and vaccinations required on the links below

It is important to make this initial appointment as early as possible - at least 8 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Most travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because travel vaccination is not included in the services provided by the NHS and therefore is a chargeable service.

Yellow Fever Centres - The Locks Road practice is an authorised centre for yellow fever immunisations by appointment with the practice nurses. They are available to all patients, including those not reqistered with this group practice.
Physiotherapy Clinic - Appointments are available via your doctor who will refer you if appropriate.
Registered patients aged 16-75 who have not been seen for three years may request a consultation.
Registered patients aged over 75 years who have not been seen in the previous 12 months may request a consultation.

(If you are unable to attend the surgery for these checks because of your medical condition a home visit may be arranged.)
