Out of Hours
On weekdays between the hours of 6.30pm and 8.00am, and all day and night at weekends and on bank holidays, services are commissioned by Hampshire Primary Care Trust.  If you wish to see an emergency doctor, phone Locks Road on 01489 583777 or Whiteley on 01489 881982

Test Results
If any tests are taken, it is your responsibility to telephone the surgery for the results. Please only telephone between 1.00pm and 3.00pm. If you wish to discuss the results, please request a routine telephone conversation with your GP or Nurse.

Cancelling your Appointment
If you are unable to attend an appointment with one of the doctors or nurses, please telephone either surgery.

registrationHOW DO I REGISTER ?

To register as a patient ask at reception for details.  Health services reforms mean that patients will no longer be registered with an individual doctor but with the practice and can be seen at both surgeries.

You may, however, nominate your preferred doctor and a note of this will be made on your records.  We cannot, however, always guarantee the availability of your first choice of doctor.  Newly registered patients will be invited for a new patient health check at this time.


Refusal Of Application For Inclusion In The List Of Patients Or For Acceptance As A Temporary Resident

The practice will only refuse an application if it has reasonable grounds for doing so, for example if the applicant does not live in the practice area.  Applicants will be notified in writing of the refusal and the reason for it.


PRACTICE BOUNDARIES

The practice areas are defined on maps within both surgeries. These have been agreed by the Hampshire Primary Care Trust.  Patient paper medical records are held by the surgery in whose catchment area their address is located.
